Photo/Video Release
I hereby grant permission to Mothers Of Murdered Columbus Children (MOMCC) to use my photograph/video on its website, in printed publications and video productions without further consideration, and I acknowledge the MOMCC’s right to crop, edit or treat the photograph/video at its discretion. I also acknowledge that MOMCC may choose not to use my photo/video now, but may do so at its own discretion later.
I also understand that once my image is posted on the website, the image can be downloaded by any computer used on or off campus. Therefore, I agree to indemnify and hold harmless from any claims the Board and leadership of Mothers Of Murdered Columbus Children & All employees, volunteers and members of Mothers Of Murdered Columbus Children.
Mothers Of Murdered Columbus Children reserves the right to discontinue use of photos/videos without notice.
GENERAL RELEASE WAIVER
I hereby acknowledge that I am voluntarily participating in trade activities organized by the Mothers of Murdered Columbus Children’s (MOMCC) Trendy Trades Program. I understand that these activities involve inherent risks, including but not limited to bodily injury, property damage, and death, which may occur as a result of my participation. Therefore, I agree to the following terms:

RELEASE AND WAIVER OF LIABILITY: I hereby release and forever discharge the MOMCC’s Summer Trades Program organization, its directors, officers, agents, employees, and volunteers (collectively, the "Released Parties"), from any and all liability, claims, demands, and causes of action that I may have now or in the future, arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, whether caused by the negligence of the Released Parties or otherwise.

ASSUMPTION OF RISK: I understand and acknowledge that my participation in the trades activities organized by the MOMCC’s Summer Trades Program involves inherent risks, including but not limited to bodily injury, property damage, and death. I voluntarily assume all risks associated with my participation in these activities.

TRANSPORTATION: While MOMCC is not offering transportation, due to the nature of the trades program, the youth might be offered transportation, and you agree to work out these details with instructors and voluntarily assume all risks associated with my participation.

INDEMNIFICATION: I agree to indemnify, defend, and hold harmless the Released Parties from any and all claims, demands, and causes of action, including attorneys' fees and costs, arising out of or related to my participation in the trade activities organized by the MOMCC’s Summer Trades Program organization.

MEDICAL TREATMENT: I authorize the MOMCC’s Summer Trades Program organization, its agents, employees, and volunteers, to obtain or provide medical treatment for my child in the event of any injury, illness, or other medical condition that may occur during my participation in the trade activities.

ACKNOWLEDGEMENT OF UNDERSTANDING: I have read this trade waiver, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue the Released Parties for any injury, death, or damages that may result from my participation in the trade activities organized by the MOMCC’s Summer Trades Program organization.
By signing the line below, I acknowledge that I have read and fully understand this trade waiver, and I voluntarily agree to its terms.
Student Agreement
Your youth will receive the Trade experience, be part of fun exploration experiences paid for by Mothers of Murdered Columbus Children, receive a free t-shirt, a completion gift item, and earn a bonus of up to $200. This payment will be made at the end of the program, pending the completion of the program, as outlined in this agreement. Failure to complete the program will result in not receiving the $200 payment.

CLOSING CEREMONY: The youth must participate in the closing ceremony, where they will present their final project.

FINAL PROJECT: They must complete their final project, as deemed by the instructor, and share it with the leadership of MOMCC or they will not be eligible for the stipend.

HOURS: Youth must complete 25 of the 30 hours between July 8th and August 9th.

RERIMANDS: If given three reprimands, youth will could be dismissed from the program and not receive the stipend. The first reprimand will be verbal. With the second, the parent will be notified. With the third, there will be a parent-teacher conference with a leader from MOMCC and the student will be subject to removal and no stipend.

CELL PHONES: There will no cell phone use during any portion of the class, unless directed by the instructor for class purposes.

UNIFORM: Youth must wear the provided T-shirt uniform to class.

TARDY: Students must be on time. If they arrive 10 mins after the scheduled start time, they will be considered tardy. IF they collect five tardies, there will be a parent-teacher conference and they will be subject to removal.

SHOES: Close-toed shoes (no sandals or crocs) must be worn for all sites.
